You are being invited to volunteer to participate in a research study. Research studies include only people who voluntarily choose to take part.

        The purpose of this research is to find out the effect of daily journaling on mental and physical health outcomes in cancer patients undergoing radiation therapy.

        • Below is a brief summary about the procedure, risk, benefits, and sponsors of the study:

        • If you participate in this study, you will be offered the opportunity to read and comment on participants’ blogs and/or to post your own blogs regarding your experience with cancer using a secured website.

        • The study presents no more than minimal risk. The only identifiable information needed from you is for creating an account for accessing the secured website. There is a risk of loss of confidentiality if your information or your identity is obtained by someone other than the investigators, but precautions will be taken to prevent this from happening. Any identifying information will be destroyed following publication of the data. All other data collected by the research study does not contain any other information that can be used to identify any subject. Furthermore, data gathered by the research will be stored in an encrypted database, which is stored in a secured server protected by the firewall of College of Information Sciences and Technology.

        • There is no guarantee that you will benefit from this research. Medical science may gain a greater understanding about the psychosocial needs of cancer patients.

        • No sponsor, institution, or companies are paying HMC/PSU for the research to be done.

        • This section is about health information that can be traced to you.

               o The content of your daily journals and your activities on the website will be stored in a secured database at University Park campus of Penn State University.

               o In the event of any publication or presentation resulting from the research, no personally identifiable information will be shared.

        You have the right to ask any questions you may have about this research. If you have questions, complaints or concerns or believe you may have been harmed from participating in this research, you should contact Dr. Heath Mackley at 717-531-1523 or Dr. Michelle Newman at (814) 863-1148. If you have questions regarding your rights as a research subject or concerns regarding your privacy, you may contact the research protection advocate in the HMC Human Subjects Protection Office at 717-531-5687. You may call this number to discuss any problems, concerns or questions; get information or offer input.

        You do not have to participate in this research. Taking part in the research study is voluntary. Your decision to participate or to decline the research will not result in any penalty or loss of benefits to which you are entitled.
